Adult Medical Nursing
Skilled nursing is provided by a primary care nurse who is experienced in or specializes in adult medical surgical nursing, behavioral health nursing, pediatric nursing, rehabilitative nursing and wound care. The primary care nurse admits the patient to service, performs comprehensive assessments, establishes the patient care plan, and coordinates services with the patient, family and other disciplines involved in the care plan. While the patient is on service, the primary care nurse makes regular evaluations of the patient's progress, acts promptly when there is a change in the patient's condition, communicates regularly with the physician, identifies patient needs and arranges referrals to other disciplines as needed. Other skilled nursing functions include educating the patient and family about condition and disease processes, developing the home health aide care plan and supervision of the home health aide.
Behavioral Health Nursing

Wound Care
The wound care program at New England Home Care is designed for patients suffering with chronic or acute wounds caused by a variety of etiologies. Our multidisciplinary team, lead by our certified wound care nurse, develops a patient specific treatment plan, taking into account all aspects of care. The diverse patient population may be recently discharged from the hospital or referred by their physicians for home wound care services. Our team assesses the patient and their wound to provide a patient centered approach to wound healing. This plan is designed for each patients' specific needs. Our goal is to provide the best possible environment for wound healing while caring for the entire patient.
Pediatric Nursing
The nurses at New England Home Care are specialists in the field of pediatric, obstetrical and newborn nursing. The primary care nursing staff is extremely skilled in physical assessment and management of the acutely ill and the chronically compromised child, and the high risk Antepartum/Postpartum mother. Our Pediatric nurses have experience and/or certification in multiple specialty areas including neonatal/child resuscitation, high risk OB, breastfeeding education, newborn/NICU nursery, inpatient pediatrics, childbirth education, NICU/Pedi flight transport, PMT restraint certification, ENPC certification and NCC inpatient obstetrics.
Physical and Occupational Therapy and Speech Language Pathology
The goal of the rehabilitation department is to help patients achieve their optimal level of functional independence and safety within their home and community.
Social Work
Medical Social Worker Services are provided by a Medical Social Worker (MSW) team experienced and knowledgeable about Health Care Systems and those Community Resources available to meet patient needs, based on the presenting problems expressed by the referral source and the patient. The MSW also facilitates and improves family communication related to the patient's situation. Options for financial assistance or support will be explored with the patient. Short term counseling is provided to the patient so as to identify the support and community resources needed to maintain the patient's optimal level of functional independence and safety within their home and community.
Paraprofessional Services
Our Home Health Aide services are delivered under the supervision of a registered nurse, or licensed therapist. Our home health aides are certified and require continuing training and education units yearly. An aide is assigned to a case when there is a specified need for personal care.
