Patients
Home health care is a great alternative to extended hospital stays. Often times, hospitalized patients who elect home health care can be discharged early. This reduces hospital admissions, promotes a quicker recovery and results in happier patients.

Who qualifies for home care services?
To be eligible to receive NEHC skilled home care services, patients must meet several criteria:
- Must have a homebound status.
- Must be under the care of a physician.
- Must require additional care following a recent hospital or nursing home discharge.
- Must have short-term health needs.
If the patient is in the hospital what should be done?
- The patient or family member should tell the physician and the hospital social worker that they want a Home Care referral.
- The patient or family member may request New England Home Care, Inc. Everyone has the right to choose the home care agency they want providing the services. If you do not specify a particular agency, such as NEHC, the physician or hospital social worker can randomly select a service for you.
- Before the patient is discharged from the hospital the patient or family member should verify that a referral to the home care agency of choice has been made.
- NEHC will admit a patient within 48 hours of the referral, unless otherwise specified.
Referral Services
If your patients or clients prefer to live independently at home rather than a nursing home or need help transitioning from hospital or nursing facility to home, NEHC is the answer.
For more information or to make a referral please call our Central Intake Line 800.950.1004. Office hours are from 8:00 a.m. - 5:00 p.m. Weekends, holidays and after hours are supported by an on call system for each office with medical supervision by a registered nurse.
